Standard licenses apply to many clinics based on ownership and services. Exempt status may apply in specific situations — for example certain physician-owned practices. The correct classification depends on your ownership structure and what you offer patients.

What to prepare

  • Proof of location approvals (Certificate of Use, zoning)
  • Medical director and clinical staffing plans
  • Policies, floor plans, and application fees as required
